The Supervision Report clause requires a party, often a contractor or service provider, to regularly submit detailed reports on the progress and status of supervised work or activities. Supervision Report a. The supervision report includes documentation that non-licensed PATH staff responsible for clinical practices are supervised by a Licensed Professional of the Health Arts (LPHA) as defined in 25 Texas Administrative Code §412.327
b. Documentation shall include:
i. Name of LPHA and license; and
ii. Name of non-licensed PATH staff supervised.
c. Grantee shall submit documentation of supervision as requested by System Agency.
Supervision Report a. No later than October 16, 2019, Grantee shall send by email to System Agency documentation that non-licensed PATH staff responsible for clinical practices are supervised by a Licensed Professional of the Health Arts (LPHA) as defined in 25 Texas Administrative Code
