Summary Sheet. 1. This section of the proposal must include a fully-completed copy of the Summary Sheet included with this RFP (Appendix B ). 2. Provide the name, title, experience and qualifications of the personnel who will be assigned to the project. 3. Provide the resumes of the Management Contact with the City and the Project Manager (person responsible for day- to-day work on the project).

Provide an organizational chart showing key staff, roles and responsibilities including sub-consultants and support staff.
