Submit Paid Receipts to the ADMINISTRATOR. A paid receipt from a licensed service provider, stating the type of service and the date provided, must be submitted along with the AGREEMENT number and the odometer mileage of the VEHICLE on the date the service was provided, to the ADMINISTRATOR, at the address shown below, within sixty (60) days from the date of the covered service or repair to be eligible for payment.
