Submission and Report Preparation. Notify CalSCHLS Regional Center staff upon completion of each survey administration per the guidelines provided at registration. CALIFORNIA HEALTHY KIDS SURVEY (CHKS) ADMINISTRATION • Grades and Schools. Survey Grades 3 through 12 as appropriate within the District. Provide current student enrollment figures for all schools by grade level. • Parent Consent.
