SMP. If participating in the SMP, the SFA will comply with all requirements of 7 CFR Part 215 – Special Milk Program and if applicable Part 245 – Determining Eligibility for Free and Reduced Price Meals and Free Milk in Schools.

SMP. G6 HSO: G4 HSO; G4 HSO; G3 HSO; G4 Responsible for providing operational and strategic leadership in the delivery of clinical services. Ensures clinical governance systems are in place to audit, monitor and review clinical outcomes. Works collaboratively with all key stakeholders to build a culture of engagement, innovation and continuous learning and improvement.
SMP. If participating in the SMP, the SFA will comply with all requirements of 7 CFR Part 215 – Special Milk Program and if applicable Part 245 – Determining Eligibility for Free and Reduced Price Meals and Free Milk in Schools. Operate a nonprofit milk service. Claim reimbursement only for milk as defined in 7 CFR 215 and in accordance with the provisions of Sections 215.8 and 215.10. Submit claims for reimbursement in accordance with 7 CFR 210.10 and procedures established by the SA. Maintain a financial management system as prescribed by SA. If electing to serve milk at no charge only to needy children under 7 CFR 215.7(d)(2), the SFA agrees to serve milk free to all eligible children, at times that milk is made available to non-needy children under the SMP. Determine eligibility for free milk for children who are not categorically eligible in accordance with current USDA income eligibility guidelines. Comply with all requirements applicable to offering free milk in 7 CFR 245 - Determining Eligibility for Free and Reduced Price Meals and Free Milk. Retain the individual applications for free milk submitted by families for a period of three years after the end of the fiscal year to which they pertain, except that, if audit findings have not been resolved, the records shall be retained beyond the three year period as long as required for the resolution of the issues raised by the audit. Adhere to the Policy Statement below when providing free milk as described for the provision of free school meals of this Agreement except for the provision requiring verification of applications. Abide by the hearing procedures set forth in 7 CFR 245.7 and the nondiscrimination practices set forth in 7 CFR 245.8.
SMP. The AAA shall:
(1) provide group education and one-on-one assistance to Medicare beneficiaries. One-on-one assistance can be in-person, over the phone, on the internet, via postal mail, or through email;
(2) provide in-person group education sessions to teach beneficiaries to prevent, detect, and report Medicare fraud, errors and abuse;
(3) develop and maintain a partnership network to deliver SMP services;
(4) use media and other outreach plans to reach underserved or hard to reach populations;
(5) recruit, train, and retain a sufficient and effective workforce to provide high quality education and inquiry resolution;
(6) ensure all SMP staff and volunteers receive training on the following topics:
(A) the mission and function of the SMP Program;
(B) Medicare basics;
(C) healthcare fraud, errors and abuse; and
(D) effective skills in conducting educational presentations and outreach;
(7) implement the SMPs VRPM policies developed by HHS;
(8) document and progress towards improving the following PMs:
(A) PM1. Total number of active SMP team members;
(B) PM2. Total number of SMP team member hours;
(C) PM3. Number of group outreach and education events;
(D) PM4. Estimated number of people reached through outreach and education events;
(E) PM5. Number of individual interactions with or on behalf of, a beneficiary;
(F) PM6. Cost avoidance on behalf of Medicare, Medicaid, beneficiaries, or others;
(G) PM7. Expected Medicare recoveries;
(H) PM8. Additional expected Medicare recoveries;
(I) PM9. Expected Medicaid recoveries;
(J) PM10. Additional expected Medicaid recoveries;
(K) PM11. Actual savings to beneficiaries; and
(L) PM12. Additional savings;
(9) submit program data using HHS data system by the 15th of each month; and
(10) submit all semi-annual and grant end reports at the request of the DHHS Program Manager.
