Sick Leave Application. Application for sick leave for a period of more than three (3) consecutive work days but not more than five (5) consecutive work days, shall be made in such manner as the Employer may from time to time prescribe and when the application for sick leave is for a period of more than five (5) consecutive work days, it shall be supported by a certificate from a medical practitioner.

Sick Leave Application. Sick leave with pay shall be granted to all EMPLOYEES covered by this MOU in accordance with this section. Sick Leave Application. To justify each use of sick leave, the employee will complete a signed, written statement explaining the nature of the illness or other reason for taking sick leave on forms devised by Management. If medical attention is required, the employee must submit a certificate from a licensed physician stating the nature of illness to be eligible for sick leave benefits. Management reserves the right to withhold benefit payments to any employee submitting a false claim, and may take disciplinary action, including discharge. Falsification of either the written signed statement or the physician’s certificate are grounds for disciplinary action, including discharge.

Sick Leave Application. Accumulated sick leave may be used only in the event of illness or disability of the employee or the employee's dependent child, or, in the case of critical illness, a member of the employee's immediate family who permanently resides with the employee. "Dependent child" includes an unmarried person who is (a) dependent upon the employee; and (b) younger than twenty (20) years of age (except in the case of a child who is mentally or physically handicapped); and (c) the natural child, adopted child, step-child or xxxxxx-child of the employee or of the employee's partner.

Accumulated sick leave may be used only in the event of illness or disability of the employee or the employee's dependent child, or, in the case of critical illness, a member of the employee's immediate family who permanently resides with the employee. Sick leave may also be used for, personal mental health & wellbeing challenges, including dealing with extreme stress, overcoming personal hardships that make it challenging to work, caring responsibilities, illness, accident, or temporary disability of a parent, and any other emergency caring responsibilities, subject to the approval of the employee’s Supervisor or designate.

Sick leave days are to be used for absence caused by personal illness, injury, medical disability (including childbearing), poor health, or an emergency caused by family illness where no reasonable alternative is available to the employee. Documentation from the attending medical provider may be required for each absence of five (5) days or more. In the case of an employee’s illness or injury and return to duty following an absence of five (5) days or more, a statement from the treating medical provider certifying ability to return to work may be required.
