Service Sheets. The Contractor shall furnish the Authorized User with a warranty repair service sheet for all service requests upon completion of the services to include: 1. Date and time notified; 2. Date and time of arrival; 3. Description of malfunction reported; 4. Diagnosis of failure and work performed; 5. Date and time failure was corrected; 6. Charges for the service, if applicable; and 7. Name of person performing the service.
