REQUIRED INFORMATION TO BE SUBMITTED. BY THE BIDDER REQUIREMENTS YES NO a. Provide documentation and certification of participation in a program to ensure a drug and alcohol-free workplace. b. Provide a list of production and location sites. c. Provide a company resume, to document the company’s experience working with a school district or organization with multiple locations in the above-referenced service (minimum of 5 years). d. Provide three (3) current letters of reference, consisting of existing customers (see Exhibit 5). e. Copy of current registration with the Florida Department of State, Division of Corporation to conduct business in the State of Florida. f. Provide copy of current Business Tax Receipt. Any person, firm corporation or joint venture, submitting a bid with a business location within Miami-Dade County, Florida, shall meet the County’s Local Business Tax Receipt requirements in accordance with Miami-Dade County, Florida, code. Bidder(s) with a location outside Miami-Dade County, shall meet their local Occupational Tax requirements. A copy of the license must be submitted. Non-compliance with this condition may cause the bidder to not be considered for award.
