REQUIRED FORMS & DOCUMENTS. The following forms and documents are required. Forms 1 – 5 are standard application forms in ▇▇▇▇▇▇.▇▇▇. These forms require standard information about your organization and/or signatures. Forms 6 and 7 are what you use to attach the documents your application will be evaluated on. These documents include your workplan, itemized budget, and other supporting documentation. Failure to include all documents listed below may result in your proposal being rendered ineligible: 1) SF-424 Application for Federal Assistance 2) SF-424A Budget Information for Non-Construction Programs 3) SF-424B Assurances for Non-Construction Programs 4) EPA Form 4700-4 – Preaward Compliance Review Report for All Applicants and Recipients Requesting EPA Financial Assistance 5) EPA Key Contacts Form – Applicants are encouraged to ensure that all information presented on the Key Contacts form is accurate. Information contained on the Key Contacts form may be used in the event an applicant is contacted for additional information.
