Please note: Failure to provide the following may result in your proposal being rejected. • Written Summary of Proposal • Completed Space Available Proposal form(s) • Proof of ownership of Property (recorded deed or fully executed purchase agreement, purchase option, or master lease for subject property) • Proof of Legal authority to enter into the proposed agreement • Proposed Floor Plan (you must show how the State’s requirements, using State standards, will fit in your proposed site) • Proposed Site Plan • Project timeline from date of executed agreement • Signed Bidder’s Acknowledgement
