Reporting to State. Contractor shall submit biannual reporting to the State regarding programming that includes the number of offenders referred by MDOC, the number of offenders successfully and unsuccessfully completing, the current programming capacity, and other information as requested by the State.
Reporting to State. Contractor shall comply with all programmatic reporting requirements established by the State. In addition, the Contractor shall provide the following reports to the State Contract Liaison, or designee:
Reporting to State. Pharmacy must report all immunizations administered to individuals under eighteen (18) years of age to the Mississippi Immunization Information eXchange (MIIX). 1
Reporting to State. 5 Pharmacy shall report the vaccines administered pursuant to this Agreement to the Wisconsin Immunization Registry within seven (7) days of administering the vaccine. The parent, guardian or legal custodian may have the person’s record excluded from the registry by completing Wisconsin Immunization Registry Exclusion (HCF 5102) and submitting the completed form to the State Vital Records Office.
Reporting to State i. Prior to administering a vaccine, Pharmacy must notify the parent, guardian, or person in loco parentis of the child of the right to object to the reporting requirements of the Michigan Care Improvement Registry (MCIR). ii. Unless the parent, guardian, or person in loco parentis of the child objects by written notice, Pharmacy must report to the MCIR, within 72 hours of administering an immunization, each immunization administered to an individual who is less than twenty (20) years of age. iii. Pharmacy may report to the MCIR an immunization administered to an individual who is greater than twenty (20) years of age. 2
