Reporting Off Work, Providing Medical Documentation and Communicating with OHS. When an employee is to be absent due to illness or injury, the employee must report off consistent with the Company's absence reporting policy, which the Company reserves the right to change. Further, in accordance with Section 13.03 above, the employee must comply with requests by the Company or its vendor for appropriate documentation to certify absences, including medical documentation. Such documentation will typically include diagnosis, prognosis, treatment plan and return to work plan. Failure to provide appropriate documentation in accordance with this Article, as determined by the Company, may result in sick pay being withheld or denied and possible disciplinary action depending on the situation. Employees are expected to report sick absences before the start of their shift barring mitigating circumstances. Further, employees must communicate with Occupational Health Services (OHS) as OHS determines necessary. More complicated or serious conditions may require more intensive and frequent communication with the employee or the employee’s health care provider and more information regarding the case may be required.
