Common use of Relocation Evaluation Clause in Contracts

Relocation Evaluation. If Community terminates Residency Agreements under this Section A, it will take all reasonable steps to transfer affected residents safely and to minimize possible transfer trauma. Before you receive a termination notice under Section 2 above, Community will prepare a relocation evaluation of your needs. Your relocation evaluation shall include recommendations on the type of facility that would meet your present needs based on your current Assessment and a list of those type of facilities that are located within a 60-mile radius of the Community. Community shall discuss your relocation evaluation with the Resident and any Responsible Party within thirty (30) days of issuing the notice of termination under Section 2.

Your relocation evaluation shall include recommendations on the type of facility that would meet your present needs based on your current Assessment service plan and a list of those type of facilities that are located within a 60-mile radius of the CommunityFacility. Community shall discuss your relocation evaluation with the Resident you and any Responsible Party your legal representative, if any, within thirty (30) days of issuing the notice of termination under Section 2.
