Recognised Test Reports. (a) A Recognised Test Report is a report fulfilling either of the following criteria: (i) a National Association of Testing Authorities (NATA) Australia or International Accreditation New Zealand (IANZ) endorsed report; (ii) a test report from an overseas laboratory that has been accredited for the relevant Standards by an overseas accreditation body that has a Mutual Recognition Agreement (MRA) with NATA or IANZ. The report must be endorsed with the logo or certification ▇▇▇▇ of the accreditation body; (iii) a "CB" test report and certificate issued under the IECEE CB Scheme; (iv) a report from a testing laboratory which has been included as part of a test report from a NATA or IANZ laboratory that is accredited for the Standards and test concerned; (v) a Finite Element Analysis (FEA) report produced by an accredited ISO9001 company, having FEA in the scope of their certification, and issued under the signature of an engineer who is a NAFEMS Registered Analyst; and (vi) contains results for all relevant clauses and sub clauses of the relevant Standard, provided that in any of (i) to (vi) inclusive, the date of issue of the Recognised Test Report must be no more than five (5) years prior to the date on which the relevant Service (whether a testing or certification service) is or will be provided. (b) TRA (or applicable TRA Associate) may (in its sole and absolute discretion) accept a witnessed manufacturer’s testing (WMT) report produced by a Client under the direct supervision of TRA (or TRA Associate). A WMT report may be considered by TRA (or TRA Associate) for a non-complex testing methodology that only requires the completion of simple inspections and/or tests such as mass, length, time or a physically observable characteristic. (c) The Client agrees that where Recognised Test Reports are not provided by the Client, tests as a prerequisite to certification services will be carried out by TRA (or TRA Associate) at the Client's cost in accordance with these Regulations (and, without limitation, Regulation 2).

  • Loop Testing/Trouble Reporting 2.1.6.1 Telepak Networks will be responsible for testing and isolating troubles on the Loops. Telepak Networks must test and isolate trouble to the BellSouth portion of a designed/non-designed unbundled Loop (e.g., UVL-SL2, UCL-D, UVL-SL1, UCL-ND, etc.) before reporting repair to the UNE Customer Wholesale Interconnection Network Services (CWINS) Center. Upon request from BellSouth at the time of the trouble report, Telepak Networks will be required to provide the results of the Telepak Networks test which indicate a problem on the BellSouth provided Loop. 2.1.6.2 Once Telepak Networks has isolated a trouble to the BellSouth provided Loop, and had issued a trouble report to BellSouth on the Loop, BellSouth will take the actions necessary to repair the Loop if a trouble actually exists. BellSouth will repair these Loops in the same time frames that BellSouth repairs similarly situated Loops to its End Users. 2.1.6.3 If Telepak Networks reports a trouble on a non-designed or designed Loop and no trouble actually exists, BellSouth will charge Telepak Networks for any dispatching and testing (both inside and outside the CO) required by BellSouth in order to confirm the Loop’s working status. 2.1.6.4 In the event BellSouth must dispatch to the end-user’s location more than once due to incorrect or incomplete information provided by Telepak Networks (e.g., incomplete address, incorrect contact name/number, etc.), BellSouth will ▇▇▇▇ ▇▇▇▇▇▇▇ Networks for each additional dispatch required to repair the circuit due to the incorrect/incomplete information provided. BellSouth will assess the applicable Trouble Determination rates from BellSouth’s FCC or state tariffs.

  • Progress Reporting a) RECIPIENT must satisfactorily demonstrate the timely use of funds by submitting payment requests and progress reports to ECOLOGY. ECOLOGY reserves the right to amend or terminate this Agreement if the RECIPIENT does not document timely use of funds. b) RECIPIENT must submit a progress report with each payment request. Payment requests will not be processed without a progress report. ECOLOGY will define the elements and frequency of progress reports. c) RECIPIENT shall use ECOLOGY’s provided progress report format. d) Quarterly progress reports will cover the periods from January 1 through March 31, April 1 through June 30, July 1 through September 30, and October 1 through December 31. Reports shall be submitted within thirty (30) days after the end of the quarter being reported. e) RECIPIENT must submit within thirty (30) days of the expiration date of the project, unless an extension has been approved by ECOLOGY, all financial, performance, and other reports required by the Agreement and funding program guidelines. RECIPIENT shall use the ECOLOGY provided closeout report format.

  • Contract Reporting The Contractor shall provide the Department the following accurate and complete reports associated with this Contract.

  • Expert Reports Without prejudice to the appointment of other kinds of experts where authorised by the applicable arbitration rules, the tribunal, at the request of the disputing parties, may appoint one or more experts to report to it in writing on any factual issue concerning environmental, public health, safety or other scientific matters raised by a disputing party in a proceeding, subject to such terms and conditions as the disputing parties may agree.

  • Cost Reports Seller, at its expense, shall, or shall cause the Seller Entities to, prepare and timely file all terminating and other cost reports required or permitted by law to be filed under the Medicare and Medicaid or other third party payor programs and the State Health Agency for periods ending on or prior to the Effective Time, or as a result of the consummation of the transactions described herein (“Seller Cost Reports”). If requested by the Seller Entities, Buyer shall include the applicable Seller Entity’s Medicare bad debts that are returned from collection agencies subsequent to the Closing Date on Buyer’s cost report for the respective period to which the Medicare bad debt relates. The Seller Entity shall provide detailed supporting information, as required by Medicare regulations, for the Medicare bad debt account amounts to be included on Buyer’s Medicare cost report. Buyer shall forward to Seller any and all correspondence relating to the Seller Cost Reports within five (5) business days after receipt by ▇▇▇▇▇. Buyer shall remit any receipts of funds relating to the Seller Cost Reports promptly after receipt by ▇▇▇▇▇ and shall forward to Seller any demand for payments within three (3) business days after receipt by ▇▇▇▇▇. Notwithstanding anything to the contrary in this Agreement, Seller shall retain all rights to the Seller Cost Reports including any amounts receivable or payable in respect of such reports or reserves relating to such reports and all liabilities relating thereto. Such rights shall include the right to appeal any Medicare or Medicaid determinations relating to the Seller Cost Reports. Seller shall retain the originals of the Seller Cost Reports, correspondence, work papers and other documents relating to the Seller Cost Reports. Seller will furnish copies of such cost reports to Buyer upon request.
