Sponsor, Envestnet and Envestnet Manager’s Disclosure Brochures: You acknowledge that you have received a copy of the Sponsor’s Disclosure Brochure or Combined Disclosure Brochure, as applicable, and each of Envestnet’s Form ADV Part 2A and each applicable Envestnet Manager’s Form ADV Part 2A or other brochure meeting the requirements of Rule 204-3 under the Adviser’s Act (each, an “Advisory Service Provider Disclosure Brochure”). Contact your financial advisor if you would like another copy of the Disclosure Brochure, Combined Disclosure Brochure or the applicable Advisory Service Provider Disclosure Brochures.
