Qualification for storage. 1. By signing the Customer’s Written Declaration, the Child’s Mother shall consent to the collection of her peripheral blood and testing it for IgM and IgG toxoplasmoza gondii antibodies, HBs-Ag, anti-HBc, anti-HCV, anti-HIV 1, 2, syphilis test, anti-CMV IgM, anti-CMV IgG, HBV DNA, HCV RNA, HEV RNA, HIV RNA. Due to the nature of the Contract, the Child’s Mother authorizes the Child’s Father – if he is a party to the Contract – to collect, browse, review her blood test results. 2. Since the purity level and other components of the acquired Biological Material depend on natural factors beyond the control of PBKM and the personnel acquiring the Material, PBKM cannot guarantee that the Biological Material shall meet all the qualification criteria determined pursuant to the Contract nor that the acquisition shall be sterile. 3.
