PSA TESTING. The City is to pay for a PSA test for all members and spouses of members once every twenty-four (24) consecutive months. LIMITATIONS
(i) Services normally paid through any provincial hospital plan, any provincial medical plan, WSIB, other government agencies or any other source.
(ii) Services provided in a chronic care or psychiatric hospital, chronic unit of a general hospital, health spa, or when a patient is confined to a nursing home or home for the aged and receives Ontario government assistance.
(iii) Dental care (except as outlined under "Benefits").
(iv) Rest cures, travel for health reasons, insurance examinations or services or supplies for cosmetic purposes.
(v) Charges for hospital accommodation.
PSA TESTING. Protein Specific Antigen (PSA) screening. One (1) screening test per 12 months for men age 40 and over, covered at one hundred percent (100%) and not subject to the deductible.
PSA TESTING. The employer agrees to pay premium for PSA testing through Green Shield Canada.
