Provider’s Services. The Provider is a provider network that agrees to provide health care services for those employed by the Employer and who have chosen to enroll in the Foundation Health, LLC provider network Direct Primary Care plan. The services offered by the Provider will include but not be limited to the generally accepted appropriate care that is administered in a family practice office setting. In addition, the Provider agrees to provide nutritional counseling, disease management, referral to specialty care providers and general wellness counseling as appropriate for each patient. All services available through Provider are listed on Attachment A attached hereto and incorporated herein by reference. Services are provided through physicians and non-physician practitioners duly licensed to practice medicine and to otherwise provide such services. Employer acknowledges that Provider does not provide comprehensive health insurance coverage nor does it enter into a contract of insurance with patients. Provider will not be responsible for the cost of any medical services received by any patient outside of the Service Facility. Employer is responsible for managing the eligibility and the cost sharing arrangement between employer and employee (if any). Foundation Health LLC makes no guarantee that the direct care membership fee is tax deductible. Foundation Health recommends that you seek professional legal and accounting advice for the tax consequences of all employee benefits plans. Employer or Provider may cancel this agreement with 30 days’ notice, while complying with all abandonment provisions, regulations and rules. However the individual patient (employee) may continue the services and contract directly with Foundation Health, LLC, if both parties agree to do so. In addition, the Provider’s patients may at any time cancel their membership with Foundation Health. Provider does not require any waiting periods for employees nor will services be withheld based on pre-existing health conditions. Enrolled employees may seek treatment at the service facility located at the below address or at any other future Foundation Health service facility.
Provider’s Services. As used in this Agreement, the term “Services” shall mean a package of ongoing primary care services, both medical and non-medical , and certain amenities, which are offered by Provider, and set forth in the Patient Agreement.
Provider’s Services. This Agreement applies to Provider’s practice locations established as of the Effective Date of this Agreement, which shall be updated from time to time upon agreement of the parties. In the event Provider begins providing services at other locations (either by opening such locations itself, or by acquiring, merging or coming under common ownership and control with an existing Provider of services that was not already under contract with Optum to participate in a network of health care providers), such additional locations will become subject to this Agreement 30 days after Optum receives notice.
Provider’s Services. Services Routine Maintenance Services shall be performed in accordance with the provisions in Attachment 1 “Technical Specifications”. These services consist of the regular maintenance of the facilities and improvements located with the various Districts and lots shown on the attachments to the original Agreement including: Attachment 2, “Diagrams” and Attachment 3; “Photographs”. Routine Maintenance Services shall comply with the provisions of Attachment 4 to the Agreement, the “Contra Costa County Integrated Pest Management Policy” in the Agreement. Routine Maintenance Services are the same as those for FY 2020-21 and include but are not limited to: • Weeding, Cleaning, Removal • Litter Pick-up • Check for irrigation problems/coverage – Weekly • Prune & Trim shrubs and ground cover as needed • Spot spray weeds in paved surfaces as needed, use marker dye • Fire weed abatement • Empty trash cans and provide liners (Transit Hub Only) B. PROVIDER’S Routine Maintenance Services and frequency of maintenance shall be as shown on Table 1, “Facilities & Frequency of Maintenance by Service Area” attached hereto and made a part hereof.
Provider’s Services. 2.1 Services to be provided by the PROVIDER shall include, but are not limited to, those described or specified herein. The services described or specified shall not be deemed to constitute a comprehensive specification having the effect of excluding services not specifically mentioned.
Provider’s Services. The Service Provider will:
1. Evaluate and determine the Candidate's eligibility for the J-1 Program according to the U.S. Department of State's current regulations.
2. Evaluate the Candidate's English level.
3. Send the application package to be fully completed by the Candidate.
4. Reformat the Candidate's resume to the American standard.
5. Arrange interviews with potential Host Companies.
6. Prepare the Candidate for the interviews with potential Host Companies.
7. Negotiate the conditions of the training/internship: stipend, means of transportation and housing, if provided by the Host Company.
8. Prepare Form DS-7002, "Training/Internship Placement Plan", according to the Host Company's specifications.
9. Complete and secure requisite signatures on Form DS-7002.
10. Prepare the Candidate's file to obtain the Form DS-2019, "Certificate of Eligibility for Exchange Visitor Status", which is a prerequisite for obtaining the 31 Visa.
11. Prepare the Candidate with regard to their interview at the U.S. Embassy / U.S. Consulate.
12. Send the Candidate his/her medical insurance card valid for the duration of the DS- 2019.
13. Assist the Candidate upon arrival by providing the "Candidate Arrival Package" with information regarding housing, Social Security, bank account, etc.
Provider’s Services. PROVIDER shall provide professional audiology and hearing health services to Adults and Minors in accordance with any applicable requirements of South Africa. Hearing health service will include a hearing screening and review of audiogram diagnostic and report. PROVIDER will refer all Adults and Minors requiring hearing aid devices, cleaning care for ongoing maintenance to an Audiologist; offer Adults and Minors or guardians with assistance with submitting claims to the Medical Aid Schemes.
Provider’s Services. In the event Organization’s Board develops concerns regarding Provider’s services, or if Provider develops concerns regarding the performance of the Executive Director, the Parties agree to follow the Notice of Concern Process, attached hereto as Exhibit 2.
