Provider Specifications Clause Samples
The Provider Specifications clause defines the specific requirements, standards, or qualifications that a service provider must meet to fulfill their contractual obligations. Provider Specifications i. The following requirements shall apply to the Contractor, the provider, and the provider staff:
a. Professional staff shall be licensed, registered, certified or recognized under California scope of practice statutes. Professional staff shall provide services within their individual scope of practice and receive supervision required under their scope of practice laws. Licensed Practitioners of the Healing Arts (LPHA) include:
i. Physician;
ii. Nurse Practitioners;
iii. Physician Assistants;
iv. Registered Nurses; v. Registered Pharmacists;
