Proposer’s Experience Form. Proposers shall be required to complete and submit the “Proposer’s Experience Form”, included herewith. A qualified Proposer must have requisite experience in financial services, and must be actively engaged in this field for a period of no less than five (5) years at the time of proposal submission. Proposer shall demonstrate their experience on the Proposer’s Experience Form. Proposers shall provide no less than five (5) representative references completed within the past five (5) years. All blank spaces of the form shall be fully completed in legible ink or typewritten.
