The Proposal Components clause defines the specific elements that must be included in a proposal submitted under an agreement or solicitation. Proposal Components. The following documents are to be returned as part of your proposal submittal: Execution of Offer - Signed (Section 7) Respondent’s Questionnaire (Section 8) Pricing (Section 2.3) Respondent’s Qualifications / References (Section 2.4) Attachment A – Non-Collusion Statement- Signed Seven (7) hard copies of the complete proposal response is required. NOTE: The original signature on the ONE required hard copy will serve as the official signature of record for all seven copies. Copies 2-7 may be reproductions.
Proposal Components. The following items must be submitted in order for the proposal to be considered:  NON-COLLUSION FORM Every proposal must include a certification of good faith, certifying that the proposal was made in good faith and without collusion or fraud. See ‘Non-Collusion Form’ attached.  TAX COMPLIANCE FORM Every proposal must include a written certification that the proposer has complied with all state laws relating to taxes, reporting of employees and contractors, and child support. See ‘Tax Compliance Form’ attached.  CORPORATE BIDDER FORM If the proposal is being submitted by a corporation the proposal must include a certification that the individual submitting the bid has been authorized to bind the corporation. See ‘Certificate of Corporate Authority’ attached.
Proposal Components. 4.2.1. Cover letter The Proposal must include a letter of transmittal attesting to its accuracy, signed by an individual authorized to execute binding legal documents on behalf of the Proposer. The cover letter shall provide the name, email address, telephone [and facsimile numbers] of the Proposer along with the name, title, email address, telephone and facsimile numbers of the executive that has the authority to contract with the City. Each Proposer shall make the following representations and warranty in its Proposal Cover Letter, the falsity of which might result in rejection of its Proposal: “The information contained in this Proposal or any part thereof, including its Exhibits, Schedules, and other documents and instruments delivered or to be delivered to the City, is true, accurate, and complete. This Proposal includes all information necessary to ensure that the statements therein do not in whole or in part mislead the City as to any material facts.” Specify if the Proposal includes any Proposer’s trade secrets that must be shielded in case the City/County is subject to the Freedom of Information Act (FOIA). Specific requirements for trade secrets can be found in Subsection 6.4 (4). 4.2.2. Executive Summary (Maximum [X] pages) Summarize the major factors or features of the Proposal, including any conclusions, assumptions, and generalized recommendations the Proposer desires to make. It should be designed specifically for use by individuals that may not have a technical background. It should provide an overview of the Plant(s), summary of pricing options, the project timeline(s), and include a summary of the firm’s experience with similar projects.
