Program Description Summary a. The Rapid Rehousing Services for Individuals will provide a minimum of three (3) months to a maximum of twenty-four (24) months of assistance to support people experiencing homelessness with securing and transitioning into permanent housing and sustaining that housing. Rapid Rehousing Services for Individuals will be coordinated and made available through the Coordinated Entry System (CES) which prioritizes housing resources for those with the highest vulnerabilities and length of homelessness. Rapid Rehousing Services will work with CES Access Points, street outreach teams and emergency shelter programs, to support participants in identifying appropriate housing options through housing navigation services and financial assistance support. The program will assist participants experiencing homelessness in accessing the most appropriate services and resources across the System of Care, including behavioral health, healthcare, benefits and mainstream services, housing, navigating application and enrollment processes, and providing advocacy and support as necessary. The program will follow Housing First principles and incorporate evidenced-based approaches such as Motivational Interviewing, Critical Time Intervention, Progressive Engagement, trauma-informed care, harm reduction and risk management, to address barriers to housing and economic stability. The program is to be implemented in a manner that increases equitable service access across Orange County, regardless of where a person is experiencing homelessness. b. The Rapid Rehousing Services for Individuals shall include the following services at minimum: i. Intake and assessment to engage and enroll persons referred by CES who would benefit from a rapid re-housing intervention to overcome their homelessness, understanding the history of participation in other homeless service assistance programs and collection of needed demographic information from Participants. Program must be able to assess and re-evaluate the Participant’s service needs and make recommendations to appropriate and eligible housing and/or supportive services that best meets the Participant’s needs. ii. Housing-focused case management support participants with locating housing options that meet the participant’s needs. Case management services will be focused on furthering the progress towards the goals and objectives as outlined in the Individualized Housing and Service Plan. iii. Program Description Summary a. The CONTRACTOR will primarily be responsible for developing collaboration between public and private agencies, including but not limited to COUNTY Departments, homeless service providers and community-based organizations. The CONTRACTOR will ensure that the Individual CES is accessible to and serves individuals who are experiencing homelessness or at risk of experiencing homelessness in Orange County. The CONTRACTOR for the Individuals CES for the North, Central and South SPA of Orange County will be required to fulfill four (4) key competency areas – (1) leadership, (2) system support and guidance, (3) regional coordination and (4) system partnerships. i. Leadership – CONTRACTOR shall provide leadership to ensure the CES operates with an efficient, clear and consistent process across respective SPA(s) and in collaboration with all of the SPAs in Orange County in accordance with federal, state and local regulations, policies and procedures. ii. System Support and Guidance – CONTRACTOR shall provide system support and guidance around the CES core competencies. iii. Regional Coordination – CONTRACTOR shall assist in facilitating SPA level meetings to increase regional engagement and alignment with CES and to disseminate information at a regional level. The primary goal of these meetings will be to facilitate care coordination to support participants with connections to appropriate housing resources. The SPA level meetings allow for discussions and problem- solving strategies that reflect the SPA’s unique needs and resources. iv. System Partnerships – CONTRACTOR shall be tasked with developing collaboration across public and private agencies within the North, Central and South SPA to support those experiencing homelessness and special subpopulations such as veterans and transitional aged youth. This may be done through the development of a SPA specific Referral Network that includes representatives from different sectors within the System of Care. b. The CONTRACTOR will match grant funds with no less than 25 percent of funds or in-kind contributions from other sources. Program Description Summary. Contractor’s State ESG Rapid Rehousing Program shall annually serve sixty (60) unduplicated individuals for a total of twenty-three (23) unduplicated households with rapid rehousing services as defined in 24 CFR §576.104-106 (Program). Program shall be administered in an equitable manner by providing culturally responsive services and have multicultural staffing and case managers to engage and guide underserved participants throughout their participation and enrollment in the Program.

Emergency shelters should be equipped to serve Participants who may have extensive behavioral, physical or medical issues, and as a result, may face significant barriers to housing. Program Description Summary. PROGRAM is an emergency shelter operation that is designed to provide unsheltered homeless individuals in Orange County with safe shelter, supportive services, health services, housing support services, and access to support to move individuals out of homelessness and into permanent housing opportunities. The overall purpose of the PROGRAM is to connect homeless persons to permanent housing opportunities and resources to maintain housing stability and self- sufficiency. The CONTRACTOR will be responsible for the day to day operation of the PROGRAM on an ongoing basis and will be required to adhere to the guidelines established in the MOPSP.. Access to bed availability will be handled through a reservation system. No walk- ins for the PROGRAM or services will be permitted.

HDAP Services for Families shall operate in accordance with Housing First Principles as defined in the California Welfare and Institution Code (WIC) Section 8256 and further outlined in All County Letter (ACL) 19-114. The purpose is to engage and serve justice-involved high-utilizers individuals of the corrections systems, specifically those who have been incarcerated four of more times within the last year, and ensuring they receive reentry support services upon their transition to the community. Through the services, a provider will be able to engage and assess individuals that need and are eligible for comprehensive Care Coordination services as referred by the Administrator. Services through the Program are inclusive of comprehensive case management, connections to supportive services, and housing stabilization/navigation services to individuals with the goal of reducing recidivism. b. The Program will become an integral part of the Care Plus Program and function as a closed Corrections Cohort service provider access point throughout Orange County, to support Participants in accessing available and appropriate resources. c. The Program will incorporate evidenced-based approaches such as motivational interviewing, critical time intervention, trauma-informed care, harm reduction and risk management, to address barriers within the criminal justice system. The program is to be implemented in a manner that increases equitable service access across the County. d. The Program must include the following services at minimum: i. Targeted outreach and engagement to Participants of the Care Plus Program, working to build relationships that offer Care Coordination and support to access services and assistance programs, mainstream services, and other programs. ii. Contractor will provide linkages post-custody programs via “warm- handoffs” to community care upon release with no lapse in the continuity of services. iii. Intake and assessment to determine the history of participation in other public service assistance programs and collection of required demographic information from Participants.
