Person interested to receive a plastic card shall submit to the Bank appropriate written application or by means of remote service channels of the Bank (in form set by the Bank), and to enter/ credit to appropriate bank account irreducible balance (subject to existance/request of such) and fee for the card issuance and annual commission rate according to rates set by the Bank.
