Probationary Interim Report. 13.7.1 A Formal mid-year conference for first-year probationary personnel is to be completed by the second Friday in December. 13.7.2 The Evaluator shall prepare two sets of Evaluation Report forms at the beginning of the evaluation period. Entries shall be made on both sets of forms; one set to be used for the Interim Assessment Report for probationary-first year employees; one set to be used for the final Evaluation Report.
