Personal Leaves of Absence. Personal and other unpaid leaves that are not considered Family and Medical Leaves under the Lifespan System-wide Policy, including School Involvement Leave as described in the Lifespan System-wide Policy, may be granted by the Program Director. Personal Leaves of Absence. SRD includes the time an employee is on "leave", if the employee is on a: • personal leave of absence with pay; or • personal leave of absence without pay which is less than 15 working days; or • personal leave of absence without pay which is more than 15 working days and which was started on or after April 15, 1993 -- only that portion which was taken during the period from April 15, 1993 to August 31, 1997. • prepaid leave of absence, under the enhanced leaves of absence policy dated April 16, 1993, greater than eight (8) weeks.
Personal Leaves of Absence. LONG-TERM
11.04.01 A personal leave of absence exceeding one hundred and eighty (180) days will be a long-term leave.
11.04.02 Employees will make their requests, in writing, to their immediate supervisor at least fourteen (14) days in advance of the commencement date of the requested leave.
11.04.03 Requests will be approved in order of seniority among those on hand at the time of granting, subject to Article 11.
Personal Leaves of Absence. Employees shall submit a written request for a personal leave of absence to her/his supervisor (or her/his designate) who will submit the written request to the DTC Manager (or his/her designate). Such written request will indicate the proposed period of the leave and the reasons for the leave. Written requests will be submitted at least thirty (30) days in advance of the proposed commencement of the leave. Personal leave requests shall be granted at the Company’s discretion, subject to its operational requirements. The Company shall not unreasonably deny an employee’s request for a personal leave of absence provided that such leave may be arranged without undue inconvenience to the normal operations of the Company. Full-time seniority employees who have been granted personal leave shall continue to accrue seniority during the approved leave. Benefit coverage will be maintained for an eligible full-time employee who has been in receipt of benefits to the end of the calendar month in which the leave commenced. Benefit coverage will be maintained for an eligible full-time employee who has been in receipt of benefits for a period of up to six (6) months during a leave of absence, when such employee takes a leave after being requested by the Company to take such leave of absence during a slow business period instead of being laid off.
Personal Leaves of Absence. The Employer in its sole discretion may grant a leave of absence without pay to any employee for legitimate personal reasons. Such leave shall be unreasonably denied. Prior to the granting of any personal leave pursuant to an employee shall use all vacation time to which she is entitled. All leaves of absence shall be weeks in advance of the required leave. This requirement may be waived the sole discretion of the Executive Director. The Executive Director shall notify the employee in writing, at least two weeks in advance of the required leave, whether or not the leave of absence has been granted. Compassionate leave may be granted at the discretion of the Executive Director to an employee to a maximum of three days per year, without pay, for reasons which are unavoidable, extraordinary or on the grounds of compassion. No leave may be taken without formal approval of the Executive Director or her designate. Full-time employees will be entitled to fifteen days of paid discretionary leave annually and all Part-time employees be entitled to five days of paiddiscretionary leave annually to be used to cover absences related to urgent personal matters, including but not limited to illness, injury or medical emergency or an illness, injury, emergency other matter concerning an individual described in of the Employment Standards Act, provided that: no more than five days entitlement shall be carried over from one fiscal year to the next; and a medical certificate may be required, at the sole discretion of the Executive Director where: an employee has a higher than average rate of absenteeism; or in any event, where an employee is absent three or more consecutive days. Employees who have been granted a leave of absence pursuant to this Article will not be considered to be laid-off and their seniority shall continue to accumulate during each absence to a of three months. Employees who have been granted a leave of absence pursuant to this Article will not engage in gainful employment while on any such leave or utilize a leave of absence for purposes other than those for which it was granted. If an employee does engage in gainful employment while on such leave of absence or utilizes the leave of absence for purposes other than those for which it was granted, she may, at the sole discretion of in accordance with
Personal Leaves of Absence. 11.5.1 A Unit Member may be eligible for personal leave of absence, for a period not to exceed one (1) year, upon written application by the Unit Member, recommendation of the Unit Member’s appropriate administrator, and approval of the Employer.
11.5.2 Said leave of absence shall be without pay. A Unit Member on leave of absence under this personal leave of absence section in excess of thirty (30) calendar days shall not, from the date the leave starts, accrue seniority, sick leave or other fringe benefits granted to regular Unit Members.
11.5.3 A Unit Member must submit a request for personal leave of absence a minimum of thirty (30) days in advance in order to provide a reasonable opportunity to find a substitute employee for the duration of the Unit Member’s leave of absence, if required.
11.5.4 Failure by the Unit Member on personal leave of absence to return to duty or to have notified the Employer and have been granted an extension, at the expiration of the approved leave period may be considered abandonment of position and the Unit Member may be terminated for cause.
11.5.5 Upon receipt of the Unit Member’s notification to return, the Unit Member shall be placed in a vacant position in the Unit Member’s previous program.
11.5.6 A Unit Member desiring to return to duty prior to the expiration of the approved personal leave period shall provide the Employer with a written request to return, specifying the date of availability. Upon receipt of the request to return and the approval of the Employer, the Unit Member shall be placed in a vacant position in the Unit Member’s area of credentialing.
Personal Leaves of Absence. 20.01 If circumstances permit, the Company may grant a personal leave of absence without pay, to an employee who requests same. A request for such leave shall be in writing stating the reasons at least one (1) month prior to the desired commencement date of the leave, except in extenuating circumstances. If the Company grants a leave of absence, then it shall confirm the terms of the leave in writing.

11.03.01 A short-term leave of absence will consist of more than thirty (30) calendar days but will not exceed one hundred and eighty (180) calendar days.
11.03.02 Employees will make their requests, in writing, to Workforce Planning at least fourteen (14) calendar days in advance of the commencement date of the requested leave.

Personal Leaves of Absence. 25.01 The Employer may, in its discretion, grant a personal leave of absence without pay and without loss of seniority for up to a maximum of ninety (90) calendar days, to an employee who has successfully completed his probationary period, for a good and sufficient personal reason and subject to the requirements of the Employer’s business. The Employer will provide a response to the employee's written request for a personal leave of absence within five (5) working days of the receipt of the employee's written request.
25.02 Personal leave of absence may only be requested by an employee who has successfully completed his probationary period, and shall be requested by the employee in writing as far in advance as possible and shall set out the length of leave of absence requested and the purpose of the leave. The Employer's decision to grant in its discretion an employee's request in writing for a personal leave of absence in accordance with this Article will not be unreasonably denied.
25.03 Any personal leave of absence granted by the Employer shall be confirmed in writing and shall set out the length of leave of absence granted, the purpose of the leave and terms, if any, on which is granted. At least two (2) weeks or period remaining thereof, of eligible vacation time not yet taken or booked, must be taken prior to the leave of absence. A copy of the employee's written request for a personal leave of absence and the Employer's response shall be provided to the Unit Chairperson upon request.
25.04 A personal leave of absence will not be granted for the purpose of working elsewhere or benefiting from receipt of money from other employment sources.
25.05 An employee returning from an authorized leave of absence will be returned to his previous conditions of employment held at the time of being granted an authorized personal leave of absence.
Personal Leaves of Absence. Personal leaves of absence may be granted 7 to regular status nurses at the Hospital’s discretion, for up to twelve (12) 8 consecutive weeks, independent of any leaves provided for by law. 10 A leave of absence must be requested in writing and approved by the nurse’s 11 clinical manager.
