PERFORMANCE WORK STATEMENT. If the Contractor shall fail to report services in the CCB data collection software application by the 15th of the month for previous month’s services the resulting penalty of a deduction in the amount of $10.00 for each day data is late will be made. The penalty shall be reflected on the A-19 invoice or noted in a supporting email. Due to the nature of this project’s potential contribution to statewide prevention system development, if the Contractor shall fail to report services in the CCB data collection software application by the 15th of the month for two (2) or more consecutive months during the contract period, the resulting penalty may be exclusion from future opportunities to continue participation in the Monitoring and Participatory Evaluation Pilot Targeted Enhancement Project.
