PERFORMANCE AND MONITORING. A. The Contractor agrees to report all occupational illnesses and injuries which are diagnosed during the contract period to the DOL , utilizing the form entitled “Physician’s Report of Occupational Disease”. B. The DOL may make informal visits of reasonable frequency and duration with advanced notice for the purpose of program monitoring and evaluation. Such visits may include the examination of a reasonable number of patient files and records. C. The DOL shall monitor the Contractor's overall program performance, inform the Contractor of any specific program deficiencies, and make requests for corrective action when necessary. (1) The Contractor will be responsible for submitting an annual program report that complies with the guidelines specified in Attachment A, Annual Program Report Guidelines Occupational Health Clinics. The report must be submitted to the DOL on or before August 1or thirty one (31) days following the termination of the contract, whichever comes first. (2) The Contractor agrees to cooperate with the DOL by providing any other performance reports or information requested by the DOL for the purpose of evaluating the activities funded by this contract. A time schedule for regular reporting will be determined by the DOL. E. The DOL shall have the right to terminate this agreement in accordance with the General Conditions. F. The Contractor further agrees to abide by all terms, conditions, and specifications detailed in the DOL-approved response to the Request for Proposal for an Auxiliary or Occupational Health Clinic.

CTDOL shall monitor UConn Health’s overall program performance, inform UConn Health of any specific program deficiencies, and make requests for corrective action when necessary. d. UConn Health will be responsible for submitting an Annual Program Report that covers clinic activities and patients seen between July 1, 2022 and June 30, 2023. The report shall comply with the following guidelines: a. Total number of patient visits (including information as to the nature of the visit, i.e., initial visit, follow-up, etc.). b. Total number of injury and illness reports submitted to the Department of Labor during FY 2023. c. Any epidemiologic information of note, including the identification of high-risk industries, occupations, or workplaces. d. Description of industrial hygiene visits, on-site exposure assessments/workplace evaluations conducted (listing should include type of company or industry – the specific company name does not need to be included). e. Activities that involved the training of occupational health professionals. f. Education related to the use of the surveillance system. g. Any special projects undertaken. e. PERFORMANCE AND MONITORING. 7.1 The Parties shall monitor compliance with this IAA and the Implementation Plans. 7.2 The Parties will as part of the Periodic Reviews detailed in clause 11 review waste composition, volumes, all waste streams and such other matters as may be agreed between the Parties in order to identify compliance with this IAA or variations to performance that may be required in the future.
PERFORMANCE AND MONITORING. 3.8.1 The Provider will deliver a safe, secure and professional Cash & Security service by qualified staff licensed by the Security Industry Authority (part of the Home Office). 3.8.2 Within the bounds of its authority and control, the Provider shall deliver the service to the Client as specified in Schedule A. 3.8.3 The Provider will accommodate variations to the agreement, within reason, by the Client. 3.8.4 The Provider will accommodate all reasonable requests from the Client for documentation relating to the service provided to the Client, where it is not commercially sensitive.
PERFORMANCE AND MONITORING. 8.1 For absolute clarity, these arrangements with KAL are a partnership, and not a formal contract. 8.2 An Operational Management Group comprising of KAL senior leadership and Council officers will meet regularly to provide a continuing overview of the effectiveness of the partnership in achieving these outcomes. 8.3 Given the nature of the work and the outcomes we are seeking to achieve, a mix of qualitative and quantitative reporting is appropriate. 8.4 A summary of progress being made against the agreed outcomes and measures should be included in a KAL annual report to the Council.
PERFORMANCE AND MONITORING. Khuda Bakhsh Oriental Public library will supply to the Ministry of Culture, Government of India each quarter of the year regular information in respect of achievement of the targets set out in Annexure-II enclosed with this Agreement. This informatiqn, . together with the Annual Reports / Audited Accounts and any further reports/documents, the Khuda Bakhsh Oriental Public Library prepares in relation to the progress against its own policies, will be used to monitor the yearly performance. Also for monitoring the performance of the Khuda Bakhsh Oriental Public Library will be undertaken at the end of the financial year 2014-
PERFORMANCE AND MONITORING. 2.1 Standards for the Performance of Delegated Activities developed and adopted by Health Plan are published in the Provider Manual and incorporated herein by reference. Physician Group agrees to perform the activities delegated to it in a manner consistent with those Standards applicable to activities for which Physician Group has been delegated. 2.2 Physician Group acknowledges Health Plan’s responsibility to monitor Physician Group’s eligibility for delegation according to the Delegation Criteria and performance of the delegated activities according to the Standards of Performance. Physician Group agrees to cooperate with Health Plan’s monitoring of Physician Group’s eligibility and performance of delegated activities. 2.3 Health Plan shall conduct regularly scheduled audits to determine Physician Group’s continued compliance with the Standards. Physician Group shall cooperate with Health Plan and its designated agents in the performance of those audits, including but not limited to, the provision of reasonable access during regular business hours to the Enrollee inquiry files, credentialing files, clinical and medical records of Health Plan Enrollees and non-Enrollees as applicable and reasonably necessary to evaluate Physician Group’s performance of activities delegated to it. Cooperation shall include, but not be limited to, an annual evaluation and quarterly meetings between Health Plan and Physician Group staff 2.4 Physician Group agrees to provide Health Plan with periodic reports on delegated activities performed by Physician Group. The report shall be in a form and contain such information as shall be agreed upon between the parties. 2.5 Physician Group agrees to use its best efforts to take whatever corrective actions are identified by Health Plan through the audit review process as reasonably necessary.
