On September 30, 2015, the World Health Organization (WHO) released their “Guideline on when to start antiretroviral therapy and pre-exposure prophylaxis for HIV, which was followed in 2016 by the Consolidated Guidelines on the use of ARV drugs for treating and preventing HIV infection.” This guideline expands the eligibility criteria of life-saving treatment to all persons living with HIV (PLHIV) and highlights new developments in HIV combination prevention, including pre-exposure prophylaxis (PrEP). The WHO guideline is transformative to achieving epidemic control3. Currently in Nicaragua, the ART start criteria is CD4 count under 500 which requires multiple visits to determine eligibility, delays and losses in ART initiation. This has been identified as the main bottle neck to link patients to care and treatment. In addition, diagnosis is late, 40% of patients admitted had CD4 counts under 200 cells/mm3. ROP 2016 guidelines4 recommended rapidly adopting the new World Health Organization (WHO) guidelines on treatment for all (also referred to as Test and Start) and a mandate for alternative service delivery models that will improve retention and adherence. PEPFAR endorses the 90-90-90 targets by 2020 and has pivoted to focus on doing the right things in the right places right now. Addressing stigma and discrimination and engaging the community are prerequisites for a sustainable and successful response to HIV and essential to achieve the 90-90-90 targets. All implementing mechanism in Central America, including PrevenSida need to be adjusted to move immediately to Test and Start through the adoption and implementation of the new WHO guidelines, including those related to differentiated service delivery. Activities must reflect knowledge of the main gaps and barriers to reach targets established in the ROP 2016, reflecting Test and Start priorities and a new service delivery model with defined outcomes and activities to support the achievement of those outcomes.

They also participated in developing facility training materials on how to extract patient numbers from the MoH Open MRS system to fill the patient section of the WAOS form. To strengthen the management and appropriate use of HIV commodities in ART sites, we collaborated with AIDS Control Program partners to develop an ART SPARS tool and training materials. The ART SPARS supervision tool has four indicator domains: ART patient management, ART stock management, traceability of ART commodities, and ordering and reporting quality. We started piloting the tool, and training and implementation will follow in the next quarter. Like EM SPARS, ART SPARS will be implemented in all public and PNFP ART sites by the 400-plus existing MMS.

PEPFAR. Community Systems Strengthening protocol document has been prepared and shared with NACO and final approvals are pending. Fortnightly meetings with NACO and other stakeholders have been ongoing. Standard Operating Systems (SOPs) for Community-Led Monitoring (CLM) were prepared and shared with NACO, final approvals are awaited. A total of 160 community leaders at the district level were identified and enrolled for the activity. Regular training and activities with the community leaders were ongoing. A stakeholder meeting with NACO was organized and conducted in March. A one-day workshop with Telangana Community leaders was conducted in March in Hyderabad. The PD Telangana SACs and USAID key officers were present at the event (virtually) Alongside the workshop, a social media campaign was managed to focus on the Transgender Rights Day.
