PCP Change Form. If a change form is used, by the contractor, the contractor shall immediately provide the PCP Change Form to an enrollee wishing a change, if such request is made in person, or by mail if requested by telephone or in writing. The contractor shall mail the form within three (3) business days of receiving a telephone or written request for a form.
