Common use of Parent Certification Clause in Contracts

Parent Certification. I certify by use of this certificate, that I am exercising my choice of caregiver for my child(ren) and that the possible child care options including licensed provider, licensed home provider and informal provider have been explained to me. Based upon the current rate schedule, the total amount of subsidy that the child(ren) qualify for, minus my assessed fee, will be paid directly to my child care provider. I understand that I will not be reimbursed for registration fees or any additional charges above the agreed upon rates. To receive payment for child care, my provider must present the Attendance Rosters by the 3rd working day of the calendar month, signed by the caregiver and myself. I also understand that payments made to my provider may be reported to the IRS as income received, for tax purposes.
