Parent Certification. I certify by use of this certificate, that I am exercising my choice of caregiver for my child(ren) and that the possible child care options including licensed provider, licensed home provider and informal provider have been explained to me. Based upon the current rate schedule, the total amount of subsidy that the child(ren) qualify for, minus my assessed fee, will be paid directly to my child care provider. I understand that I will not be reimbursed for registration fees or any additional charges above the agreed upon rates. To receive payment for child care, my provider must present the Attendance Rosters by the 3rd working day of the calendar month, signed by the caregiver and myself. I also understand that payments made to my provider may be reported to the IRS as income received, for tax purposes.
Parent Certification. I understand that I can choose who will provide child care for my child. I understand that if I choose someone to come into my home to provide child care, that I am the sole employer of this person. I understand that as the employer, I am responsible for paying minimum wage and benefits to my employee and I may have other responsibilities to my employee. I understand that it is my obligation to find out what I am responsible for as an employer and make sure I am doing what is required of me as an employer, including but not limited to paying all applicable Federal and State employment taxes required to be paid by me as an employer. I understand that any child care benefit for which I am eligible may only cover a portion of my child care costs. Parent Certification. On the date the Proxy Statement contained in the Form S-4 is first mailed, Parent shall deliver to the Company a certificate dated as of the Company Mailing Date signed by its President and its Chief Financial Officer, in their capacities as officers of Parent, that the representation and warranties of Parent and its subsidiaries contained in the Agreement are true and correct on the Company Mailing Date (except to the extent that they relate to an earlier time, in which case they shall have been true and correct as of such earlier time) and that with respect to information relating to Parent and its subsidiaries, the Form S-4 does not contain any untrue statement of a material fact or omit to state a material fact required to be stated therein or necessary in order to make the statements therein, in light of the circumstances under which they were made, not misleading.
