Overarching Goals. Grantee shall: 1. Implement a CSC program for early psychosis identification and service provision in accordance with Attachment A-1 Coordinated Specialty Care Implementation Manual; 2. Establish a dedicated First Episode Psychosis (FEP) Team; 3. Provide a Certified Peer Provider in accordance with the standards defined in Texas Administrative Code, Title 25, Part 1, Chapter 416, Subchapter A, Mental Health Rehabilitative Services; 4. Adhere to all fidelity standards in Attachment A-1 Coordinated Specialty Care Implementation Manual and Attachment A-13 ONTrackNY Fidelity Protocol in the provision of CSC services; 5. Assign one or more staff responsible for tracking policy updates posted on System Agency’s identified platform and disseminating information within the organization to ensure contractor stays informed and continues receiving updated information; 6. Provide an average of 5 hours of services per month for the first 12 months an individual is enrolled in the program (Note: The service period begins when an individual is enrolled in the program and applies to the first 12 months the individual receives services, even if those 12 months are not contiguous.); 7. Create an Implementation Plan that includes: a. Team members’ names, positions, credentials, and percentage of Full Time Equivalent (FTE) if not working full-time; b. Methods to address staff vacancies for the duration of this contract; c. Timeline for internal Fidelity Reviews to ensure that services are being provided in accordance with CSC standards; and d. Implementation timeline which includes benchmarks for the following: i. Date the FEP Team will be in place and fully trained; ii. At least 3 strategies that will be implemented to provide services; iii. When services will begin for the CSC enrolled individuals; iv. Team training as outlined in Section I. Grantee Responsibilities, Subsection C, Staffing; v. Any anticipated barriers to CSC service provision, and the strategies that will be used to address the barriers; and vi. Strategies for community outreach and engagement.
