Ongoing Reporting and Evaluation i. Contractor will continue data collection, data entry and reporting activities as determined by an independent evaluator contracted by the County. ii. Contractor will submit quarterly reports to the BHRS program manager that include progress-to-date, achievements and challenges and narrative specifically addressing any relevant impact the Pride Center has on the following MHSA indicators: 1. Improve timely access & linkage to treatment for underserved populations. 2. Reduce stigma and discrimination 3. Increase number of individuals receiving public behavioral health services 4. Reduce disparities in access to care 5. Implement recovery principles. iii. Contractor will meet monthly with the BHRS program manager to discuss program implementation and any proposed changes to the agreement terms. iv. Contractor shall complete and submit the year-end Attachment M – MHSA Annual Report Template, due by the fifteenth (15th) of August each fiscal year to the MHSA Manager at ▇▇▇▇@▇▇▇▇▇▇.▇▇▇.
