Offer. The undersigned, having carefully examined the site of work, the Design Plans, the Maine Department of Transportation’s Standard Specifications (Revision of November 2014) as updated through advertisement, Supplemental Specifications, Special Provisions, Contract Agreement; and Contract Bonds contained herein for construction of: State of Maine, on which bids will be received until the time specified in the “Notice to Contractors,” does hereby bid and offer to enter into this contract to supply all the materials, tools, equipment and labor to construct the whole of the Work in strict accordance with the terms and conditions of this Contract at the unit prices in the attached “Schedule of Items.” The Offeror agrees to perform the work required at the price specified above and in accordance with the bids provided in the attached “Schedule of Items” in strict accordance with the terms of this solicitation, and to provide the appropriate insurance and bonds if this offer is accepted by the Municipality in writing. As Offeror also agrees: First: To do any extra work, not covered by the attached “Schedule of Items,” which may be ordered by the Project Resident for the Municipality, and to accept as full compensation the amount determined upon a “Force Account” basis as provided in the Maine Department of Transportation Standard Specifications, Revision of November 2014, and as addressed in the contract documents. Second: That the bid bond at 5% of the bid amount or the official bank check, cashier’s check, certificate of deposit or U.S. Postal Money Order in the amount given in the “Notice to Contractors”, payable to the Municipality of Bar Harbor and accompanying this bid, shall be forfeited, as liquidated damages, if in case this bid is accepted, and the undersigned shall fail to abide by the terms and conditions of the offer and fail to furnish satisfactory insurance and Contract bonds under the conditions stipulated in the Specifications within 15 days of notice of intent to award the contract. Third: To begin the Work as stated in Section 107.2 of the Maine Department of Transportation’s Standard Specifications, Revision of November 2014, and complete the Work within the time limits given in the Special Provisions of this Contract.

The undersigned, having carefully examined the site of work, the Design Plans, the Maine Department of Transportation’s Standard Specifications (Revision of November 2014) 2014 Edition, Standard Details November 2014 Edition as updated through advertisement, Supplemental Specifications, Special Provisions, Contract Agreement; and Contract Bonds contained herein for construction of: WIN 18033.00, for the Ultra-Thin Bonded Wearing Course in the towns of Monmouth, ▇▇▇▇▇, and Winthrop, State of Maine, on which bids will be received until the time specified in the “Notice to Contractors,does do(es) hereby bid and offer to enter into this contract to supply all the materials, tools, equipment and labor to construct the whole of the Work in strict accordance with the terms and conditions of this Contract at the unit prices in the attached “Schedule of Items.” ”. The Offeror agrees to perform the work required at the price specified above and in accordance with the bids provided in the attached “Schedule of Items” in strict accordance with the terms of this solicitation, and to provide the appropriate insurance and bonds if this offer is accepted by the Municipality Government in writing. As Offeror also agrees: First: To do any extra work, not covered by the attached “Schedule of Items,” ”, which may be ordered by the Project Resident for the MunicipalityResident, and to accept as full compensation the amount determined upon a “Force Account” basis as provided in the Maine Department of Transportation Standard Specifications, Revision of November 20142014 Edition, and as addressed in the contract documents. The undersigned Bidder hereby declares that the names of all persons interested in this Bid as principals appear in the blank spaces hereinafter provided for such purpose, that this Bid is in all respects fair and without collusion, that the Bidder has examined the locations of the proposed Work, the Advertisement, the Instructions to Bidders, the Technical Specifications, UBE requirements of the Department of EO/EA of the City of Durham, the Contract Documents and bond forms, and the Drawings therein referred to and fully understands the same and agrees and accepts the terms and conditions thereof, that it is understood that the estimated quantities are approximate only and are given for the purpose of comparing Bids upon a uniform basis, and that said estimate shall in no way affect the unit prices for the Work. The undersigned ▇▇▇▇▇▇ hereby agrees to furnish at the Bidder’s cost the expense of all the necessary labor, tools, apparatus, machinery, equipment, transportation, and all other things which may be required to fully and properly perform all the terms, covenants, provisions, and agreements of the annexed Contract. Having examined the Site of the Work and all matters referred to in the Instructions to Bidders and the Contract Documents prepared by the Engineer for the Project, we, the undersigned, having carefully examined the site of work, the Design Plans, the Maine Department of Transportation’s Standard Specifications (Revision of November 2014) as updated through advertisement, Supplemental Specifications, Special Provisions, Contract Agreement; and Contract Bonds contained herein for construction of: State of Maine, on which bids will be received until the time specified in the “Notice to Contractors,” does hereby bid and offer to enter into this contract a Contract to supply all the materials, tools, equipment and labor to construct the whole of perform the Work in strict accordance with the terms and conditions of this Contract at for the unit prices listed in this Bid form in lawful money of the United States of America. We have included the Bid security as required by the Instruction to Bidders. All applicable federal taxes are included and State of North Carolina taxes are excluded from the unit prices. All Cash and Contingency Allowances described in Section 01 20 00 - Price and Payment Procedures are included in the attached “Schedule of ItemsBid Prices.” The Offeror agrees to perform the work required at the price specified above and in accordance with the bids provided in the attached “Schedule of Items” in strict accordance with the terms of this solicitation, and to provide the appropriate insurance and bonds if this offer is accepted by the Municipality in writing. As Offeror also agrees: First: To do any extra work, not covered by the attached “Schedule of Items,” which may be ordered by the Project Resident for the Municipality, and to accept as full compensation the amount determined upon a “Force Account” basis as provided in the Maine Department of Transportation Standard Specifications, Revision of November 2014, and as addressed in the contract documents. Second: That the bid bond at 5% of the bid amount or the official bank check, cashier’s check, certificate of deposit or U.S. Postal Money Order in the amount given in the “Notice to Contractors”, payable to the Municipality of Bar Harbor and accompanying this bid, shall be forfeited, as liquidated damages, if in case this bid is accepted, and the undersigned shall fail to abide by the terms and conditions of the offer and fail to furnish satisfactory insurance and Contract bonds under the conditions stipulated in the Specifications within 15 days of notice of intent to award the contract. Third: To begin the Work as stated in Section 107.2 of the Maine Department of Transportation’s Standard Specifications, Revision of November 2014, and complete the Work within the time limits given in the Special Provisions of this Contract.
