Objection Procedure. Any Settlement Class Member who does not request exclusion from the settlement and who wishes to object to the settlement must submit the objection in writing to the Court and the Parties’ Counsel, postmarked no later than 60 days from the date of mailing of the Notice. To be considered valid, an objection must include: (i) the Settlement Class Member’s full name, address, telephone number, and e-mail address; (ii) the case name and number; (iii) the reasons why he or she objects to the settlement; (iv) the name and address of his or her attorney, if he or she has retained one; (v) a statement confirming whether the Settlement Class member and/or his or her attorney intend to personally appear at the Final Approval Hearing; and (vi) the Settlement Class member’s (or his or her attorney’s) signature and date signed. If a Settlement Class member wishes to object to the settlement, he or she must not request exclusion.
