Notice – Access by Individual. CONTRACTOR agrees to notify THE COUNTY in writing within three (3) business days of any request by an individual for access to the individual’s PHI or EPHI and, upon receipt of such request, direct the individual to contact THE COUNTY to obtain access to the individual’s PHI. Upon request by THE COUNTY and within three (3) business days of the request, CONTRACTOR agrees to make available PHI and EPHI to THE COUNTY in accordance with 45 C.F.R. § 164.524.
