Non-Medically Necessary Services Sample Clauses

Non-Medically Necessary Services. Any services provided to Eligible Persons which are deemed not Medically Necessary by the Utilization Management Program may only be billed to Eligible Persons if Eligible Persons have agreed, in writing, to such charges prior to rendering such services.
Non-Medically Necessary Services. Provider shall not charge a Participant for a service that is not Medically Necessary unless, in advance of providing the service, Provider has notified the Participant that the particular service will not be covered and the Participant acknowledges in writing that he or she will be responsible for payment for such service.
Non-Medically Necessary Services. We will not provide benefits for any services that are not Medically Necessary.
