Monthly and Semi-Annual Reports. Objective: The Contractor shall meet the reporting deadlines based on the required information for Clients served under this Contract as listed below: Primary Activities / Resources: 1. By the tenth of the following month, Contractor shall submit ODHH monthly Service Delivery Report for the respective month and include the following: a. Number of hours of Case Management services provided; b. Number of New Clients; c. Number of Clients receiving Case Management services; d. Number of CI activities; e. Number of Clients receiving CI; f. Number of closed cases; g. A copy of New Client Information form and Notice to Clients of ODHH Privacy Practices, with Client’s signature form of new Clients served in the respective month; and h. Service Delivery Plan form of each case closed in the month reported. 2. Contractor shall submit a copy of all completed Service Delivery Plan forms of Clients with open cases from July 1 to December 31 by January 10th of the following year. Contractor shall submit all completed Service Delivery Plan forms of Clients with open cases from January 1 to June 30 by July 10th of the same year. 3. Contractor shall follow ODHH instructions in saving the forms, monthly report and supporting documents before sending them to ODHH as attachments. A. Short Term Support Services 1. The Contractor shall provide one or more of the following activities: a. Communication Assistance b. Advocacy Services c. Communication Strategies d. Crisis intervention e. Emergency management f. Identification and coordination of resources 2. In the event that the Contractor determines the Community Member only needs an information and referral service, the Client Advocate shall not open a new case under this section.
