Common use of Monitoring Forms Clause in Contracts

Monitoring Forms. The University’s QA surveillance, accomplished by the QA Coordinator and the Contract Manger, will be reported using the monitoring forms in Attachments 1 and 2. The forms, when completed, will document the University’s assessment of the Contractor’s performance under the Contract to ensure that the required results are being achieved. The Contract Manager will retain a copy of all completed QA surveillance forms.
