Menu Options. Participant may elect to purchase additional services (“Menu Options Services”) in addition to Program Services; however, the Menu Options Services are billed in addition to Program Services and are not included in the cost of Program Services. Menu Options Services may be billed to Participant’s personal health insurance plan (subject to Participant’s personal health insurance contract). Menu Options Services include optional services or consultations with providers comprising individual Program elements. Such Menu Options Services may include services delivered by acupuncturists; psychologists; massage therapists; hypnotists; personal trainers; physicians practicing in areas such as sleep medicine, endocrinology, or cardiology; or other services delivered by other types of providers.
