Common use of Lenses Clause in Contracts

Lenses. The Plan must cover lenses that are appropriate and necessary for the patient’s visual welfare, as determined by an optometrist or ophthalmologist. Single, bifocal and trifocal lenses must be covered, as appropriate. The Respondent will make available a selection of lenses from which the patient may choose and/or enable patients to access their material benefits from a retailer. When providing lenses, the Respondent will utilize an approved optical laboratory (i.e. authorized under all applicable State and federal laws to provide these services). The frequency for this benefit shall be once every twelve (12) months.
