Leave Forms. Copies of all blank leave forms shall be available in a designated area and accessible for employees.
Leave Forms. All requests for leave shall be submitted electronically or in writing using the appropriate form as prescribed by the Superintendent of Human Resources.
Leave Forms. Employees shall request vacation leave on forms provided by and in accordance with procedures established by the City. The City will approve vacation leaves at its sole discretion.

(Long term illness may be signed by spouse or next of kin) If a teacher must be away from his/her teaching station, it is incumbent on each teacher, with the permission of the building administrator, to arrange with a fellow teacher for his/her class to be adequately supervised during the time of absence. This arrangement is only permissible with respect to short-term absences of one class period or less or when a substitute is unavailable and the building administrator has authorized coverage of the absent teacher’s class period or periods by a peer teacher. In the event a teacher covers for another teacher, during their plan time, he or she shall be paid at the following rate: Elementary School $15.00 per hour (“Hour” as used in this section means no less than 45 minutes and no more than sixty minutes.) Middle School $15.00 per class period High School $15.00 per class period The use of teachers to cover for the absences of other teachers is discouraged and should only be used as a last resort. The District will strive to provide a substitute for all teachers when they are absent for more than one class period. 1995-96 1999-00 2004-05 2006-07 2007-08 The District will strive to provide a substitute teacher for all teachers when they are absent for more than one class period. 1990-91 1999-00 District-required workshop during the school day, the district will, when substitutes are available, attempt to ensure that substitutes are provided, and that coverage will not come from within the building.
