Key Attributes. A PMH has a number of key attributes that define how a practice can support patients, including through team-based care. Those key attributes are the following:
i. Person centred, whole-person care • Care is easily navigated and centred on the needs of the individual, family and community. • Individuals are empowered in optimal self-management and contribute to the development and assessment of the practice/clinic and community care models. • Care will be delivered in a culturally appropriate manner with recognition of social determinants of health and attention to marginalized populations.
Key Attributes. C.7.1.1 Key attributes of an accreditation listing are:
a. It must be presented in English as well as any other language of importance to the jurisdiction.
b. The currency of the listing should be identified with the latest publication date identified along with an indicative frequency of update.
c. The listing should be comprehensive, showing all currently and previously accredited programmes.
d. The listing should be available in a web based version to maximise access, and should provide users with the ability to search by education provider, geographical region or engineering discipline if more than (say) 100 programmes are listed. Web based versions should be clearly linked from the home page of the host jurisdiction’s website and the relevant Accord web page at ▇▇▇.▇▇▇▇▇▇▇▇▇▇▇▇.▇▇▇ It is recommended that the listing also include links to the website of the provider of each accredited programme.
Key Attributes. The CHFP should make clear targets around housing typologies, affordability, commercial investment and other elements that led to the creation of a dynamic, medical and medical research zone, while supporting existing residents. The CHFP should also have a funding planning component so that the broader ecosystem has an idea of how capital resources can be acquired and deployed throughout the development zone.
