Internet Transactions. If your card is a Visa branded card, enrollment in the Verified by Visa program is required after 3 Internet purchases. If your card is MasterCard branded, enrollment in MasterCard SecureCode program is required after 3 Internet purchases. In order to make transactions over the Internet merchants may request your Verified by Visa or MasterCard SecureCode password to complete the transaction. If a password is not provided the merchant may deny the transaction. If the password is incorrect, we may deny the transaction. By allowing another to use your Verified by Visa password or MasterCard SecureCode password, you are authorizing the transaction on your account. 3.8: Internet Gambling Notice: Restricted transactions as defined in Federal Reserve Regulation GG are prohibited from being processed through this account or relationship. Restricted transactions generally include, but are not limited to, those in which credit, electronic fund transfers, checks, or drafts are knowingly accepted by gambling businesses in connection with the participation by others in unlawful Internet gambling. We have elected to not offer accounts to organizations that offer or sponsor Internet gambling. Commercial accounts receiving or processing Internet gambling transactions are subject to closure. 4.0: MONTHLY STATEMENTS: The Bank will send you (or one of you) a monthly statement summarizing each transaction on your Card Account during the preceding billing period. Your monthly statement will show the total amount you owe as of the end of the billing period, including any interest charges, fees, the minimum payment due and the payment due date, which date is at least 25 days from the monthly statement closing date. It also shows your current Credit Limit. The Bank will mail or deliver your monthly statement to only one address. Preservation of your rights under federal law regarding resolution of billing errors depends upon your timely recognition of potential errors, as explained in “Billing Rights Summary” notice at the end of this Agreement. 5.0: HOW THE NEW BALANCE IS DETERMINED: The total amount that you owe the Bank at the end of the current billing period appears as the “New Balance” on the monthly statement. The Outstanding Balance at the end of the previous billing period is called the “Previous Balance”. The New Balance is determined by taking the Previous Balance and (1) subtracting any payments and credits received by the Bank during the current billing period and then (2) adding any purchases, cash advances, quasi cash transactions, balance transfers, interest charges, fees, and other adjustments.

Certain merchants may inform you of their intent to convert your Convenience Check payment to an electronic ACH debit. For mailed payments, this notification will be provided at the end time you receive a bill. For payments made in person, this notification will be provided at the time payment is made. You understand and agree that Convenience Checks are ineligible for conversion to an ACH debit and if presented by a merchant to us for payment, we are required to reject and return the entry. Using Convenience Checks for payments to merchants who have notified you of their intent to convert your check to an ACH debit may result in merchant returned item fees and/or late charges. You further understand and agree that you may not attempt to authorize a one-time advance from your Account via ACH by providing check information (such as the routing, account, and serial numbers) to a merchant or other payee in person, via the Internet or by telephone for the purpose of having such merchant or other payee electronically initiate a funds transfer. The Credit Union will not be liable for any fees you incur as a result of the current billing period Credit Union’s failure to honor a Convenience Check that is converted to an ACH debit. such Convenience Checks if, for example: • The amount of the Convenience Check would cause you to exceed your Cash Advance Credit Limit; • If you are in default; • If you did not comply with the instructions included with the Convenience Check; • If the Convenience Check appears as altered or is missing information; • The Convenience Check was used by someone other than the “New Balance” person to whom the Convenience Check was sent; • The Card has expired or was revoked; • Your Account is closed or suspended; or • The Convenience Check was converted to an ACH transaction. To stop payment on a Convenience Check, you must call us at (▇▇▇) ▇▇▇-▇▇▇▇ during regular business hours and provide us with the monthly statementfollowing information: the Account number, the payee, any check number that may be applicable, and the exact amount of the Convenience Check. If you order a stop payment on a Convenience Check orally, the order will be valid for only fourteen (14) days thereafter, unless confirmed in writing. Written stop payment orders will remain in effect only for six (6) months, unless renewed in writing. You have the burden of establishing the fact and amount of loss resulting from payment contrary to a binding stop payment. The Outstanding Balance at Credit Union is not required to notify you when a stop payment order expires. If the end Credit Union re-credits your Account after paying a Convenience Check over a valid and timely stop payment order, you agree to sign a statement describing the dispute with the payee, to assign to the Credit Union all of your rights against the payee or other holders of the previous billing period is called check and to assist the “Previous Balance”. The New Balance is determined by taking Credit Union in any legal action. You agree to indemnify and hold the Previous Balance Credit Union harmless from all costs and (1) subtracting any payments expenses, including attorneys’ fees, damages, or claims, related to the Credit Union honoring your stop payment request or in failing to stop payment of an item as a result of incorrect information provided to the Credit Union or the giving of inadequate time to act upon a stop payment request. You agree that in the event of a lost, stolen, not received or counterfeited card or fraudulent activity on your Account, you and credits received all parties given access to the Account will complete an affidavit of forgery in a form approved by the Bank during Credit Union. You also agree to assist us in determining the current billing period and then (2) adding any purchasesfacts, cash advances, quasi cash transactions, balance transfers, interest charges, feescircumstances, and other adjustmentspertinent information related to any loss, theft or possible unauthorized use of your Card, Account number or PIN and to comply with such other procedures as we may require in connection with our investigation.
