The 'Information and Forms' clause requires parties to provide necessary documents, data, or completed forms relevant to the agreement. In an emergency, where there is not time to complete the form, the Employer may authorize medical treatment by telephone and then forward Form CA-16 to the medical facility within 48 hours. Form CA-16 may not be used to authorize treatment for occupational disease or illness except if the Department of Labor’s Office of Worker’s Compensation Program authorizes such use in an individual case.
