IN PROCEDURES. Tenant will notify Landlord of Tenant's proposed move-in date and will pay any fees that are associated with the move-in procedure: Tenant will be provided Seven (7) calendar days after move-in to conduct an inspection of the premises and to note any deficiencies or inoperable appliances. If no report is filed by the Tenant within this time period, it is assumed that there are no deficiencies in the property and that all appliances are in working and acceptable condition upon move-in. It is strongly encouraged for the Tenant to conduct a thorough move-in inspection.

IN PROCEDURES. MEC/Maximus Receptionist • If the “walk-in” member/applicant drops off a change of address document such as a new utility xxxx or a note with his/her name and new address written on it, the MEC/Maximus receptionist must explain that if the member/applicant wishes to register to vote, the individual can do so at the office and can get help if needed, or the individual can take home a voter registration form. Explain that MassHealth will mail the voter registration form to the appropriate local election official if it is completed in the office. If the individual wants to register to vote at the office, provide a Massachusetts Official Voter Registration Form (if help is requested, refer the individual to an eligibility worker). If the individual asks to take a form home to fill out later, provide a Mail-in Voter Registration form. The MEC/Maximus receptionist must enter the member’s name and the reason for the visit (to report an address change) on the walk-in tracking log. The MEC/Maximus receptionist puts the reported address change document in a designated box for work to be mailed to the EDMC. • If the “walk-in” member/applicant asks to see an eligibility worker, the MEC/Maximus receptionist must record the member/applicant’s name and reason for visit on the walk-in tracking log, specifying whether the member/applicant is there for help applying, for help reapplying, for help changing an address, or for assistance with another issue. The MEC/Maximus receptionist must provide the member/applicant a Declination Form and ask the member/applicant to complete it while waiting for the eligibility worker and then hand the completed form to the eligibility worker. Eligibility Worker/Customer Service Representative (CSR) • Upon welcoming the member/applicant into the interview room, the eligibility worker/CSR asks for the completed Declination Form and reviews it. • If, in Part A of the Declination Form, the member/applicant checked off “No” or “Already registered where I live now”, no further action is needed. The eligibility worker/CSR completes Part B of the Declination Form and enters the member/applicant name, DOB and Medicaid ID on the top of the form. Tear off and hand the member/applicant Part C. • If the member/applicant did not complete Part A of the Declination Form, the eligibility worker/CSR will ask the member/applicant to complete the form, and provide assistance in doing so if requested. Customer must print the Repair Order number clearly and conspicuously on the outside of the prepaid packaging. NCS will refuse to accept, and will return to Customer, any System that does not clearly and conspicuously display the Repair Order number on the packaging. Shipping: Customer and the carrier will make the shipping arrangements. The System must be shipped in the NCS standard prepaid packaging to the address given to Customer by the technician or provided on call tag. Package the System: Customer will provide packaging if not proved by NCS. Customer will be responsible for ensuring that the System is properly packaged and Customer will bear the full risk of loss or damage for any System that is returned improperly packaged.

IN PROCEDURES. “Break-­‐In” Procedures for 4-­‐cycle GM/MerCruiser Engines IMPORTANT: The first 20-­‐hours of operation will have a dramatic affect relative to overall engine performance and expected engine life. Proper break-­‐in practices reduce the potential for excessive oil consumption. It also helps to achieve better engine performance levels for a longer period of time. FIRST 20-­‐HOURS:
IN PROCEDURES.  Driver arrives at check-in area (VSA – Vehicle Staging Area) with full tank of fuel. Driver must have proof of re-fueling within 50 miles of check-in, if circumstances allow. Contractor recognizes the need to secure fuel prior to arrival and that circumstances may dictate that fueling of vehicles outside the 50 mile radius may be required to ensure availability of fuel prior to arrival.  Vehicle goes through Quality Control and Quality Assurance screening.  Upon arrival of the Vehicle at the Contractor’s Vehicle Staging Area/Vehicle Checkpoint Area, the Vehicle will be inspected by Contractor’s designated personnel (the “Inspector”). If Vehicle fails inspection by Inspector, Vendor may be permitted a reasonable time period to bring Vehicle into compliance, provided that such time period shall be determined at the sole discretion of the Inspector in light of the circumstances surrounding the Evacuation Operation. If Vendor does not bring the Vehicle into compliance within the time period indicated by the Inspector, the non-compliant Vehicle will not be used by the Contractor for the Evacuation Operation and Vendor will not be paid for that Vehicle's Travel Time. The decision of the Inspector regarding this matter will be final and binding.  Vehicle license plate number, matching registration, matching VIN, CB/PB # and certificate of insurance, as well as driver CDL and DOT physical card are all verified with information sent upon procurement. o If driver and vehicle have not been properly procured, they may be sent to home without pay. o If the numbers do not match records given by bus operators upon procurement, vehicle may be sent home at discretion of TMS Logistics.  Driver obtains assigned bus kit including the following: o Hard case o GPS unit o Nextel 2-way phone with battery and charger o Cigarette lighter adapter/splitter o Fuel card  Fuel cards are non-transferable and may be used for business purposes while in theater only.  Driver must stay with coach throughout check-in process.  After fully checked-in, driver may not leave staging yard without notifying dispatcher.  The assigned Bus Kit number, CB/PB number and fuel card are non-transferable.
