HUD Approved Certification. For each incident where a person is claiming victim status, that person shall certify to SMHA their victim status by completing a HUD approved certification form (form HUD-50066). The person shall certify the date, time and description of the incident(s), that the incident(s) are bona fide incidents of actual or threatened abuses and meet the requirements of VAWA and this Policy. The person shall provide information to identify the perpetrator including but not limited to the name only if the name of the perpetrator is safe to provide and is known, (and any and all known aliases) date of birth, address, contact information such as postal, e-mail or internet address, telephone or fax number and other pertinent information.
