HSRI will work with the Department to understand the routine that is followed to develop an individual support plan. This includes understanding of the underlying intentions of the planning process, case manager responsibilities, and the mechanics of how the plan is compiled. Next, HSRI will identify and share with the Department particular points at which assessment information can be integrated into the planning process, and the Department will decide how the assessment results will be integrated. Finally, adjustments to the planning process will be decided upon and integrated into the supports planning process.
