Hours Reporting. Within 30 days after the end of each quarter during the Term, the Physicians will provide to the Agency an hours report with respect to the Services provided under the Contract which identifies the days Services were provided, the number of hours of Services provided each day by each Physician, including start and stop times and the total number of hours provided by the Physicians, in the form set out in Appendix 5A.

Hours Reporting. It is the responsibility of SEs to accurately track and report hours in a timely manner on a weekly basis, no later than the first business day following the end of the pay period.
