General Methods                                        Means standard deviations (SD), medians and minimum and maximums will be presented for continuous variables, the number and percentage of patients in each category will be presented for nominal and ordered categorical variables. Statistical tests with a p value <0.05 will be considered statistically significant, unless otherwise stated.
